Review & Update Student Address
Do we have your current address? Update your address by calling your child's school's main office by July 12, 2021.
Childcare Request Form
Will your child need alternate stops (daycare, relative's home, etc.) this year? It must be one location and all 5 days of the week. Forms may be picked up at your child's school, at the Transportation Office or downloaded here.
Medical Transportation Form
For children who have medical issues which prevent them from traveling to and from a bus stop safely. This form must be filled out and completed yearly. Contact Health Services (315) 435-4145 for information.
